Thank you for choosing Spine & Sport Physical Therapy!  By now, you’ve probably already scheduled your first appointment, and you may be wondering what to expect.

Your First Physical Therapy Visit

When you first enter our office you will be greeted by a friendly, cordial staff that is eager to help. If you have not already completed your new patient forms at home, we will have you complete them in our office.  Expect to spend about one hour with a caring therapist doing a detailed and thorough examination. The therapist will then explain his findings and work to establish goals and a course of action. When done with your first visit you will have a better understanding of your problem, how to fix it or manage it, and understand your role in the therapy process.

Subsequent visits

The appointments following your initial appointment will generally take 45 minutes to one hour.  Always expect one-on-one time with your therapist and to be treated with respect and dignity by our entire staff. Also, expect to have fun!

New Patient Forms

To spend less time in our lobby, we encourage you to print and complete your new patient paperwork before arriving at our office.  In addition to the following forms, please also bring (if applicable):

  • Your Prescription for Physical Therapy
  • Insurance Card(s)
  • State-issued Photo ID
  • Cash, check, credit or debit cards
